You write to me regarding names[1]. You yourself will add, when you are called to the Torah* or in your signature, a name in the Holy Tongue* which, from now on, will be your principal name — even if sometimes[2] you may use all three together, or the principal name together with one of the two you currently bear.
Another point, which is essential: you will adopt, in all of this, the conduct befitting a student of Yeshiva* Tom'chei Temimim*, and Hashem will grant you success.

לכתבו בענין השמות - יוסיף לעצמו (בעלי' לתורה, בחתימה וכו') שם בלה"ק ומכאן ולהבא יעשהו לעיקר (אף שלפעמים יוכל להשתמש בשלשתם יחד, או בהעיקר וא' מב' השמות דעתה).
ועוד ועיקר - יתנהג ככל המתאים לתמים וה' יצליחו.
